When you schedule your appointment, you should be prepared to provide the following information:

  • Your name exactly as it appears on your identification. At minimum, your ID must be government-issued and show your name in English as well as your signature and photo.
  • Your Social Security number or licensing authority/sponsor issued I.D. number.
  • The best phone numbers to reach you. If the test center needs to contact you due to an unexpected event, like weather closure, these are the numbers they will use.
  • Your mailing address. This is the address where you want your score report or other important information mailed. You must provide a street address. A P.O. box will not be accepted.
  • Title of the exam you are taking.
  • Eligibility information, if required.
  • Your email address. Just as with your phone numbers, this is for contact purposes and is often the fastest, most effective way for the test center to communicate with you.
  • Your method of payment. Cost of the exam is $64, whether you are taking both portions of the exam or just the state-specific portion.
